For individuals without insurance, we base our charges on a sliding fee scale that takes into account household income and the number of family members who live with you. To receive this kind of medical financial assistance, we’ll need proof of your household income to determine the fees for services rendered.
To qualify for the sliding fee scale, you must provide the following:
- Proof of household income, which can include:
- W2
- Last two pay stubs
- Any type of government assistance
- Food stamp award letter
- Child Support
- A list of all the members of your household
- A photo ID
