Community Health Connections partners with the Connecticut River Valley Farmworker Health Program to offer healthcare to migratory and seasonal agricultural workers and their families in our services area of North Central Massachusetts.
Through this program, our outreach team travels to local farms and can offer the following Primary Medical Care services as needed:
