CHC patients may utilize their MyChart account (online secure patient portal) to leave
non-urgent messages for their primary care providers.
Please leave a brief message and your inquiry/feedback will be answered by the appropriate Department Manager, within 1-2 business day.
*Do Not include sensitive personal infomation in the message. (ie, date of birth, SSN, medical/health information.)
