Services provided at South Gardner Community Health & Urgent Care Center; Primary Care/Family Medicine, Medical Urgent Care, Pediatrics, Dental, Adult & Child Behavioral and Mental Health, Psychiatry, Podiatry, Nutritional Counseling, Patient Services/Health Insurance Assistance, Substance Use Disorder Treatment and Lab.

South Gardner Urgent Care is walk-in and open to the public. 978-410-6111

Additional CHC Urgent Care Centers are located in Fitchburg and Leominster; Fitchburg Urgent Care @ACTION Community Health Center and Leominster Community Health & Urgent Care Center.

Using the secure patient portal MyChart App, you can now save a space in line at any CHC Urgent Care location.
