Your donation will make it possible for Community Health Connections’ Fitchburg Community Health Center to move into a new building. The state-of-the-art facility will improve patient access to services and will also house the Fitchburg UMass Family Medicine Residency Program, where we train future physicians. Community Health Connections is a not-for-profit health care organization providing medical, dental, behavioral health, and pharmacy services for all ages, and helping individuals overcome barriers to care.
