Accessible Comprehensive Treatment In Our Neighborhoods

ACTION Health Services, Community Health Connections, Inc.’s newest community health center, was created to provide access to medical, dental, and behavioral health care for people who find receiving regulary primary care difficult, in locations that are convenient for them.

ACTION Health Services provides care through the use of a mobile medical unit and a mobile dental unit, and also provides medical and behavioral health care services at land-based sites.

Our staff includes physicians, nurse practitioners, licensed practical nurses, dentists, medical and dental assistants, community health workers, and administrators.  In addition to providing clinical services, we also provide services which include health insurance application assistance, referrals to additional community services, education and case management.

ACTION is the fourth health center of Community Health Connections, Inc. serving residents of North Central Massachusetts with medical, dental, behavioral health and low-cost pharmacy services.  Other CHC, Inc. locations include Fitchburg Community Health Center, Leominster Community Health Center, Greater Gardner Community Health Center.  All sites provide comprehensive health care services to anyone seeking care regardless of income, age, sexual orientation, insurance coverage status and ethnicity.
